Welcome to the official blog of Commonwealth Care Alliance, “Dually Noted.”
Commonwealth Care Alliance is a not-for-profit care delivery organization that coordinates and delivers care for more than 5,000 low-income seniors and younger individuals with complex health care needs. This innovative model reduces hospitalizations and nursing home placements, saving costs and enhancing our members’ quality of life.
The Commonwealth Care Alliance team—from our nurse practitioners, physicians and social workers in the field to our durable medical equipment specialists and member services staff at our Boston headquarters—are dedicated to providing healthcare where and when our members need it most. For many of our patients staying healthy is hard and navigating the traditional healthcare system is even harder. Our model aims to make both attainable and seamless so our members never feel like they’re going at it alone.
This year, Commonwealth Care Alliance will embark on a new journey. We have been selected as one of three health plans in Massachusetts to participate in the One Care: MassHealth plus Medicare program for dual eligible individuals ages 21 to 64 with complex medical conditions. As an approved One Care plan, Commonwealth Care Alliance will be able to give individuals in the disability community access to enhanced health and community support services delivered through an integrated, person-directed care model.
Many of the authors you’ll read on this blog have been serving this population—what we call “dual eligibles”—for more than 10 years. We hope that by sharing our insights and experiences with care delivery, we will inspire other healthcare organizations to reimagine how they deliver care, offering a brighter healthcare future for this critical population.
