An article in the Boston Globe looks at Commonwealth Care Alliance’s (CCA) health outreach workers, as well as outreach workers at Tufts Health Plan, and describes their contributions to the success of the One Care program in Massachusetts. The article explains how health outreach workers play a critical role in ensuring the plan’s success by locating members, building relationships and making sure members have the support they need. As CCA’s Chief Medical Officer Dr. Toyin Ajayi says in the article, “The success of our approach is really predicated on our ability to engage with people.”

In addition, the piece details how CCA’s One Care program has been successful in reducing inpatient hospital stays, ER visits and costs. CCA President and CEO Chris Palmieri is also quoted in the article, explaining how CCA’s One Care approach could serve as a national model for helping individuals with complex needs.
